Endodontist Dublin
Specialist root canal treatment and retreatment by Dr. Aisling Donnelly, MSc Endodontics (King’s College London) — for complex, retreatment, and second-opinion cases.
The Institute of Specialist and Cosmetic Dentistry (ISCD) provides specialist root canal treatment in Dublin 1 for patients referred by their general dentist and for those seeking a second opinion or retreatment when a previous root canal has not resolved the problem.
Specialist endodontic treatment at ISCD is provided by Dr. Aisling Donnelly, who is registered as a dentist with the Dental Council of Ireland and practises exclusively in endodontics, working as part of ISCD’s multidisciplinary specialist team at the IFSC.
What Is a Specialist Endodontist?
A specialist endodontist is a dentist who has completed a minimum of three additional years of full-time postgraduate training in endodontics — the diagnosis and treatment of disease in the dental pulp and the tissues surrounding tooth roots — and whose practice is limited to that field. The Dental Council of Ireland’s Register of Dental Specialists currently recognises two specialties only — Orthodontics and Oral Surgery — so there is no specialist register for endodontics in the State.
The distinction from a general dentist offering root canal treatment is clinically meaningful. Specialist endodontists use dental operating microscopes for every procedure, providing illumination and magnification that cannot be replicated in a general dental setting. They also use Cone Beam CT (CBCT) imaging to diagnose complex anatomy, missed canals, and root fractures that standard X-rays do not show.
For routine root canals on straightforward teeth, a competent general dentist can achieve good outcomes. For complex cases — narrow or calcified canals, teeth with unusual root anatomy, previously treated teeth requiring retreatment, or cases where an accurate diagnosis is uncertain — specialist endodontic care makes a demonstrable difference to outcomes.
Why Do Outcomes Differ Between Specialists and General Dentists?
A comparative study published in Oral Surgery, Oral Medicine, Oral Pathology found a 5-year tooth survival rate of 98.1% for root canals performed by specialist endodontists, compared with 89.7% for those performed by general dental practitioners — a clinically meaningful 8.4 percentage point difference.
The study reviewed outcomes for 350 teeth across multiple practices and found specialist treatment to be significantly more successful when measured by tooth survival at five years. (Alley BS, Kitchens GG, Alley LW, Eleazer PD. Oral Surg Oral Med Oral Pathol Oral Radiol Endod. 2004 Jul;98(1):115-8. PubMed 15243481.)
The reasons for this difference are primarily technical. Specialists use operating microscopes for every procedure, access to CBCT imaging for pre-treatment diagnosis, and have treated a considerably higher volume of complex cases during and after specialist training. For routine teeth, the gap narrows; for complex anatomy or previously treated teeth, it is most pronounced.
For referring dentists: when a case involves calcified canals, suspected missed canals, unusual root anatomy, or a previous root canal that has not healed, specialist endodontic referral is the appropriate pathway.
What Conditions Does ISCD’s Endodontist Treat?
Specialist endodontic treatment at ISCD covers the full range of root canal procedures, including initial treatment for complex anatomy, retreatment of failed root canals, periapical surgery (apicoectomy), and diagnosis of tooth pain where the cause is unclear.
Conditions and presentations commonly referred to ISCD for specialist endodontic treatment include:
- Complex initial root canal treatment — teeth with narrow, curved, calcified, or multiple canals where standard GP treatment carries elevated risk
- Root canal retreatment — when a previous root canal has not resolved symptoms, or where periapical pathology persists on X-ray
- Periapical surgery (apicoectomy) — when non-surgical retreatment is not possible and surgical removal of infected root tip tissue is required
- Cracked teeth — diagnosis and management of cracked tooth syndrome, including advice on prognosis and treatment options
- Diagnosis of complex tooth pain — where the source of pain is unclear or referred from another area
- Vital pulp therapy — where the goal is to preserve a living pulp in a tooth with deep decay or traumatic injury, avoiding or deferring root canal treatment
What Should a Dentist Include in a Referral to ISCD?
A referral to ISCD’s endodontist is straightforward — the most useful information is a description of the presenting complaint, recent periapical radiographs, any previous treatment notes, and whether the patient is symptomatic or the referral is for an incidental finding.
ISCD accepts referrals from general dental practitioners via our online referral form or by calling (01) 504 2065. Following assessment, we provide a full written report to the referring dentist including diagnosis, treatment plan, and prognosis.
Where CBCT imaging is required for diagnosis, this can be arranged at ISCD. Standard periapical radiographs provided by the referring practice are reviewed at the consultation where available.
What to Expect at Your Specialist Endodontic Consultation
The initial specialist consultation at ISCD includes a full clinical examination, review of available radiographs, CBCT imaging if clinically indicated, a diagnosis and written treatment plan, and a transparent cost estimate before any treatment proceeds.
The consultation appointment typically lasts 30–45 minutes. Dr. Donnelly will take a full history of the presenting complaint, examine the tooth and surrounding tissues, and discuss findings clearly before recommending a treatment pathway. Some patients are seen for assessment and a treatment decision; others proceed to treatment in the same appointment where this is clinically appropriate.
Specialist consultation at ISCD is €150. Root canal treatment fees are provided following assessment.
Frequently Asked Questions — Endodontist Dublin
What is the difference between a root canal by a specialist and by my own dentist?
A specialist endodontist uses a dental operating microscope, CBCT imaging, and specialist-level training in complex anatomy. For routine cases, a competent general dentist can achieve good results. For complex cases — calcified canals, retreatment, unusual anatomy — specialist treatment has significantly higher success rates. A study in Oral Surgery, Oral Medicine, Oral Pathology found 98.1% five-year tooth survival for specialist-treated teeth versus 89.7% for GP-treated teeth.
When should my dentist refer me to an endodontist?
You should be referred to a specialist endodontist when: your root canal has not resolved symptoms; periapical pathology persists on X-ray after previous treatment; your tooth has complex or calcified canals; your dentist identifies a suspected root fracture or missed canal; or the diagnosis of your tooth pain is uncertain.
Can a failed root canal be treated?
Yes. Root canal retreatment is one of the most common specialist endodontic procedures. In most cases, the previous root filling can be removed, the canals re-cleaned and re-shaped under microscope, and the tooth re-filled and re-restored. Where retreatment is not possible, periapical surgery (apicoectomy) is an alternative. A specialist assessment will determine which pathway has the best prognosis for the specific tooth.
Do I need a referral from my dentist?
Yes. Endodontic treatment at ISCD is by referral from your dentist, who can refer you via our online referral form or by calling (01) 504 2065. If you are unsure how to arrange a referral, contact us and we will guide you and your dentist through it.
